Chart Retrieval Specialist

The Chart Retrieval Specialist will manage the day-to-day activities associated with obtaining medical records from healthcare facilities and provider offices. This includes coordinating on-site retrieval appointments, scanning and uploading records, processing incoming correspondence, maintaining accurate tracking logs, reconciling outstanding records, and following up with provider offices to ensure records are received within established timelines. This role will work closely with program leadership as well as internal departments, healthcare providers, and external business partners to coordinate retrieval efforts and meet project goals.

Responsibilities

Coordinate appointments with healthcare facilities and provider offices for on-site medical record retrieval.
Manage chart retrieval activities according to project timelines and established procedures.
Retrieve, scan, upload, organize, and document medical records using designated equipment and systems.
Process incoming mail, faxes, correspondence, and medical records, ensuring documents are properly prepared, stored, logged, and distributed.
Review medical records for completeness before forwarding them to the appropriate coding or internal team.
Reconcile received records and maintain accurate lists of outstanding medical records by provider organization.
Conduct professional follow-up with provider offices regarding outstanding records, deadlines, and required documentation.
Monitor provider communications and retrieval progress to identify accounts requiring additional outreach.
Investigate and escalate issues related to missing records, correspondence, or retrieval activities.
Maintain accurate daily and weekly production and tracking logs.
Enter, update, and retrieve information using computer systems and various office applications.
Operate standard office equipment, including computers, scanners, copiers, fax machines, and other document-processing equipment.
Assist with special projects and other duties as assigned.

Required Experience

High school diploma or equivalent required.
Previous experience in a healthcare, medical records, administrative, or document-processing environment preferred.
Strong attention to detail and ability to maintain accuracy when working with large volumes of information.
Excellent organizational and time-management skills.
Comfortable communicating professionally and confidently with healthcare provider offices and other external contacts.
Ability to manage multiple priorities, deadlines, and follow-up activities.
Strong written and verbal communication skills.
Comfortable using computers, email, Microsoft Office, scanning systems, and other data-entry applications.
Ability to work independently while also collaborating effectively with internal teams and external partners.
Ability to handle confidential and sensitive medical information appropriately.
Willingness to travel locally to healthcare facilities/provider offices for on-site chart retrieval activities, as needed.
